Cigna will no longer be sending out paper 1095-B forms as they have done in the past. City employees can access their 1095-B tax forms on My.Cigna.com.

Historically, 1095-B tax forms would be mailed to Cigna customers by February 1 to confirm Minimum Essential Coverage for the prior tax year. While the 1095-B form is not required to file taxes, many customers expect a copy.

This year, Cigna has transitioned from physically mailing 1095-B tax forms to on-demand electronic availability, as part of Cigna's corporate social responsibility initiative to reduce our paper footprint. Now enrollees can access their 1095-B tax forms anytime, anywhere via myCigna.com.

You are encouraged to visit and register on myCigna® to easily save, print, or email your electronic 1095-B for the 2022 tax year.

Paper copies of the form are available upon request and can be made via email or mail:

Email: 1095BAdvocates@Cigna.com

Mail: 1095-B Request @ Cigna
900 Cottage Grove Road
Bloomfield, CT 06152

You can also contact Cigna at 1 (855) 310-7345 or the number on the back of your ID card with questions specific to the 1095-B form.
