• Certificate of Creditable Prescription Drug Coverage. This must be provided to employees prior to October 15th of each year. (Also included in back of benefit booklet);
• Women’s Health and Cancer Rights Act;
• Newborn’s and Mother’s Health Protection Act;
• Notice of Privacy Practices.
